Register with the surgery

We are currently accepting new patients at Minchinhampton Surgery. According to new NHS guidelines, we do not need to see a form of photographic identification or proof of address to register you as a new patient. Please check your home address is within the surgery’s catchment area before registering.

Registering with the surgery

To register with the surgery:

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

We recommend that all new patients book a 20-minute new patient appointment with a GP to run through their medical history and any existing medications. It is advisable that you make sure you have plenty of stock of any of your repeat medications from your previous surgery before registering here.

Out of area patients

If your home address is outside the boundary of our catchment area, you are considered an “out of area” (OOA) patient. We recommend that all OOA patients register with a GP nearer to your home address.

OOA patients may request to register, or stay registered, at Minchinhampton Surgery but it is at the discretion of the practice whether or not to accept OOA registered patients. This decision is made on an individual basis by the GP partners.

If you would like to apply as an OOA patient please fill in an out of area application form. If you are new to the surgery, this form must be brought in with a new completed registration pack.

Temporary patients

We are able to see visitors to the area as temporary patients. Please call reception so we can complete the necessary temporary registration forms. There may be a charge for overseas visitors if there is no NHS agreement between their home country and the UK.
